W-9 Instruction
This W-9 is needed on file to provide refunds to students or guardians.
1. Print, complete and sign W-9
2. Scan the Document to the Email Provided
Option 1: Go to Notes on iPhone, click on the camera icon, select the scan document button and then email.
Option 2: Take picture and email
Option 3: Utilize any scanner and then email
Email the document/picture and the student’s name to the following emails with the subject line “GHS W-9 for “Student Name”
